Description

This Continuing Medical Education (CME) activity examines the evolving role of sodium-glucose cotransporter-2 inhibitors (SGLT2is) within the "Fantastic Five" pillars of guideline-directed medical therapy for heart failure. Participants will review the mechanisms of action, evidence from landmark clinical trials, and current international guideline recommendations supporting the use of SGLT2 inhibitors across the spectrum of heart failure, with or without diabetes. The session will discuss patient selection, initiation and monitoring of therapy, management of adverse effects, and practical strategies for integrating SGLT2 inhibitors into routine clinical practice. Through case-based discussions, participants will gain insights into optimizing cardiovascular and renal outcomes using a multidisciplinary, patient-centered approach.

Objectives

By the end of this CME activity, participants should be able to: Describe the pharmacological mechanisms and pleiotropic effects of SGLT2 inhibitors in cardiovascular and renal disease. Review the evidence from major clinical trials supporting the use of SGLT2 inhibitors in heart failure and chronic kidney disease. Explain the role of SGLT2 inhibitors within the "Fantastic Five" components of guideline-directed medical therapy for heart failure. Identify appropriate patients for SGLT2 inhibitor therapy, including those with and without type 2 diabetes mellitus. Apply current guideline recommendations for initiating, monitoring, and optimizing SGLT2 inhibitor therapy. Recognize contraindications, adverse effects, and strategies for preventing and managing treatment-related complications. Integrate SGLT2 inhibitors into comprehensive, multidisciplinary care pathways to improve cardiovascular, renal, and patient-centered outcomes.
