Description
This Continuing Medical Education (CME) session will focus on the systematic review of adverse patient outcomes, including complications, unexpected events, and mortality, with the aim of identifying preventable factors and improving clinical practice. The M&M session provides a non-punitive, educational platform for healthcare teams to reflect on patient cases, analyze contributing factors, and develop evidence-based strategies to enhance patient safety, quality of care, and professional accountability.
Objectives
                                                By the end of this CME session, participants will be able to:
1.	Case Review and Analysis
o	Present and discuss selected cases of morbidity and mortality in a structured and objective manner.
o	Identify clinical, procedural, and system-related factors contributing to adverse outcomes.
2.	Clinical and Educational Value
o	Highlight key clinical learning points and evidence-based best practices from each case.
o	Differentiate between unavoidable complications and preventable adverse events.
3.	Patient Safety and Quality Improvement
o	Apply root cause analysis and quality improvement principles to address identified gaps.
o	Develop strategies to minimize recurrence of similar events and enhance patient outcomes.
4.	Professionalism and Multidisciplinary Learning
o	Foster a culture of open communication, accountability, and teamwork in patient care.
o	Strengthen collaboration among doctors, nurses, pharmacists, and other healthcare providers in preventing adverse events.
5.	System and Policy Enhancement
o	Translate case-based insights into institutional protocols, checklists, and guidelines.
