Description

Colorectal MDT Presenting: Per rectal bleeding HPI: Known Ca Rectum patient post Neoadjuvant chemotherapy and diverting colostomy, Presented to the surgical clinic with Hematochezia associated with purulent discharge per rectum. however, stoma is active. No other complain. Paternal uncle had prostate cancer. No history of smoking or alcohol consumption. Dec 2020 had exlap + hartmans colostomy for ?perforated rectum-no tumour detected at that time.Stoma reversed March 2021 and passing stool normally. 2021- back to his job Feb 2022-blood in stool 2/12,colonoscopy extensive ulcerated circum rectal mass 10-20cm from anal verge, biopsy confirmed rectal cancer, staging scan revealed: Advanced stage disease with pulmonary nodules and liver lesions segment 2 and 7. underwent 12 cycles of chemotherapy at KNH. Patient tolerated well and performance status improved. TUMOR BOARD MDT plan of treatment and follow-up for young patient with completely resected stage IVc (T3, N1, M1c) mucinous adenocarcinoma of the transverse colon with metabolically active lung nodules.
