Description

This CME session explores how telemedicine can be effectively integrated into diabetes care to improve access, continuity, and clinical outcomes. The session will cover how digital health solutions can bridge gaps in access to care. Including applications of virtual consultations, remote monitoring, patient education, and multidisciplinary care coordination.

Objectives

Describe the role of telemedicine in improving access to diabetes care, particularly for remote and underserved populations. Identify patients suitable for telemedicine-based diabetes management and those requiring in-person care. Apply telemedicine workflows for diabetes screening, diagnosis, follow-up, and long-term monitoring. Improve patient adherence and self-management through virtual education and follow-up. Recognize limitations, risks, and medico-legal considerations in telemedicine diabetes care. Coordinate multidisciplinary diabetes care using telemedicine, including referrals to nutritionists, nurses, and pharmacists.
